    Lease Renewal Request

    If you have been requested to complete this form, your current Lease Agreement is set to expire soon. We value your residency and are hopeful that you will continue to make Kalamazoo Commons your home. We would greatly appreciate your response at your earliest convenience!
    If your unit contains multiple Tenants, ONLY ONE Tenant is required to complete and submit this form.

    We would like to offer you the following lease renewal options, effective the day after your current Lease term expires:
    **ONE YEAR Lease renewal: offered at your current monthly rent amount (not including any additional fees such as, additional parking cards, pet fees, etc.). If you choose this offering, a Lease Extension Agreement will be sent to you via email to be completed and returned.**

    ​**I DO NOT wish to renew my Lease: I will submit a notice IN WRITING, 30-days prior to the expiration of my/our current Lease term. If you choose this offering, move-out procedures will be sent to you via email, which contains a forwarding address request document, to be completed and returned.​**

    **OTHER: One or more residents will be renewing the Lease and/or one or more residents will be moving-out at the expiration of the term. If you are planning on adding new replacement resident(s), they will be required to complete the application and acceptance process. An updated Lease Agreement, which will need to be completed by Tenant(s) named on the Lease, will be required. Please type any new resident names, and any other applicable information regarding your renewal; we will do our best to accommodate.**
    Please complete this form at your earliest convenience.
